(1) You will find in this directory two forms: * cmu-irb-application-v2009-aug.doc * cmu-irb-online-consent-template-v2009-aug.doc (2) Due Mar 4: * Submit a complete project proposal (items A-M of the shorter "designing a sample survey" outline from the first week of class). * Fill out "cmu-irb-application-v2009-aug.doc" as completely and earnestly as possible. - you do not need to fill in a principal investigator (PI) in block 2 - you may list me as faculty advisor in block 2 - list all your names (in your project group) in block 3 - ignore the checkboxes for "training certificate" in blocks 2 and 3 - block 4: this is unfunded research - blocks 5-12: fill out as carefully and honestly as possible - block 13: if you have a client, list him/her/it - page 8: all in your group sign and date IF SOME OF THE ABOVE ARE ALREADY DISCUSSED IN YOUR PROPOSAL, REFER TO THE PAGE NUMBER OF THE PROPOSAL INSTEAD OF REPEATING THE MATERIAL ON THE IRB FORM, IF YOU WISH. BUT BE NEAT AND CLEARLY ORGANIZED, SO IT IS EASY FOR ME TO FIND AND CHECK-OFF ALL THE PARTS, WHEREVER THEY ARE. * Following the list of 5 items at the top of page 1: - ITEM 1: submit a consent form. Use "cmu-irb-online-consent-template-v2009-aug.doc" as a template (and adapt the language appropriately if you are not doing an online survey). - ITEM 2: submit your final questionnaire, after you have pre-tested it and edited it according to what you learned from pre-testing - ITEM 3: submit drafts of email, followup email, scripts you will use if you are using face to face or telephone methods, etc. - ITEM 4: ignore; in the interests of time we will skip formal human subjects ethics training - ITEM 5: ignore; I will act as IRB in reviewing your form ALL OF THE ABOVE MUST BE CLEARLY MARKED AND EASY FOR ME TO FIND AND READ. IF SOME OF THE ABOVE ARE ALREADY DISCUSSED IN YOUR PROPOSAL, REFER TO THE PAGE NUMBER OF THE PROPOSAL INSTEAD OF REPEATING THE MATERIAL, IF YOU WISH.
